I have read the completed application; understand the rules of OKC PAL and request that my child be admitted to membership.  I have explained the rules to my child and agree that OKC PAL will not be responsible for any accident to my child while on the OKC PAL premises or while engaged in any PAL activities away from the PAL premises.  I give my consent for photographs in which my child may appear, to be used in any way OKC PAL may use them.  I understand that by submitting this online application, I am agreeing to all the above listed items.  I understand that this online registration is applicable to all OKC PAL sports and programs in the calendar year in which it is submitted.
